დაბადების ადგილი: სოფელი ველისციხე, გურჯაანის რაიონი.

თამაზ ფხაჭიაშვილი ხელმძღვანელობდა ახალგაზრდულ სამეცნიერო კლუბს. იგი მცირე ხნით ადრეც იყო სოხუმში საბრძოლველად, ილია ჭავჭავაძის საზოგადოების ოცკაციან ჯგუფთან ერთად. დაიღუპა ტამიშის ხიდთან, სოხუმი-ოჩამჩირის ტრასასთან ახლოს. საქართველოს ტერიტორიული მთლიანობისათვის მებრძოლი აფხაზეთში.
